Why 48-Hour Dressing Matters
Once the patient is discharged, the most important dressing is the one done between Day 2 and Day 3 post-surgery. At this stage, internal healing cannot be felt by the patient—but as surgeons, we need to verify what’s happening beneath the surface.
This dressing is designed to:
Detect early signs of hematoma or seroma
Confirm that internal stitches are intact
Assess whether the skin is contracting and adhering to the chest wall
Check if the nipple is settling in the desired position
If any fluid collection, abnormal swelling, or skin detachment is detected, early action is taken immediately. Identifying complications at this stage often prevents long-term issues and revision procedures.
The Role of Compression and Internal Dressings
In most gynecomastia surgeries, especially Grades 2 and above, a double compression system is used:
An external compression garment
An internal thick dressing underneath
This layered approach ensures uniform pressure, helps with skin retraction, and keeps internal tissue planes stable. In cases with excess skin, Nipple Repositioning Support (NRS) straps are also applied. These ensure that the nipple does not shift downward as the skin contracts.
During the 48-hour dressing, these internal layers are removed, wounds are cleaned, and only the compression garment is reapplied.
Patient Condition and Dressing Procedure
In this video, the patient had undergone surgery two days prior. Mild discomfort was reported on the first day, which reduced significantly by Day 2. At the time of dressing, no pain was reported—only minor tightness from the garment.
The patient had a unilateral Grade 2 gynecomastia, but bilateral gland removal was performed to maintain chest symmetry. NRS straps were applied during surgery and are being removed during this first dressing.
In higher-grade cases, these straps are left in place for 7 days, restricting the patient from bathing. But in this case, removal is scheduled today, and the patient is cleared to shower.
