Health Creating Population Health Management; using community insight and assets to get it right

Quantitative data is not sufficient to drive the right actions.

Datasets need to be accompanied by community narratives if they are to be interpreted accurately. When the conversation with communities is enabled to develop and deepen it creates fertile ground for community-led solutions to emerge, supported by services. At its best, this can lead to lasting community Health Creation.

In this video, you will hear in detail the journey one leading ICS – Lancashire and South Cumbria – is taking to establish Health Creating PHM as business as usual. It also explores with a diverse panel what needs to happen for this to become the predominant approach across all ICSs.
