Medical Animation: Fontan Operation

Fontan Operation: The Fontan Operation is the final of the three stages of congenital heart surgery to provide patients who have a single ventricle, like Hypoplastic Left Heart Syndrome (HLHS), with separation of circulation that more closely matches that of people with two ventricles. It follows the Bidirectional Glenn Shunt, which connects the superior vena cava to the pulmonary artery. The goal is now for all the venous blood returning from the body to go through the lungs first, before returning to the heart to be pumped back out. This involves connecting the inferior vena cava to the pulmonary artery. This can be done in a few different ways, but is illustrated here with what is called an “extracardiac Fontan” and also shows the addition of a “fenestration.” The IVC is disconnected from the right atrium, and connected via a tube graft to the pulmonary artery. At this point all of the venous return from the body goes directly to the lungs, before returning to the heart and getting pumped out via the single ventricle.
