This Case Presentation ‘sharing’ webinar focused on TMJ (temporomandibular joint) dysfunction and jaw pain, featuring Julie Shillabeer R.Ac, a seasoned practitioner with 20 years of experience in acupuncture. She is an expert in treating TMJ and jaw pain, with a background in acupuncture, motor point therapy, and sound healing. She emphasized her extensive training, including courses with Mary Elizabeth Wakefield, which shaped her approach to facial acupuncture and TMJ treatment.
Understanding TMJ and Jaw Pain
• TMJ is a complex joint connecting the jawbone to the skull, crucial for functions like speaking and yawning.
• Common symptoms include jaw pain, clicking or popping sounds, difficulty opening/closing the mouth, and facial asymmetry.
• Stress, clenching (bruxism), poor posture, dental work, trauma, and overuse of jaw muscles
Perspectives on Treatment
• Western Medicine: Focuses on structural and functional disorders, often using mouth guards, Botox, and surgery.
• Traditional Chinese Medicine (TCM): Views TMJ dysfunction as an imbalance in the flow of qi and blood, emphasizing emotional stress and organ system health.
Treatment Approaches
• Acupuncture: Julie discussed the importance of individualized treatment plans, including motor point therapy and sound healing.
• Home Care: Emphasized the need for patients to engage in self-care practices to complement treatments.
Case Study
The case study presented by Julie focuses on a 57-year-old female patient named Lisa, who has a history of competitive sports and presents with TMJ dysfunction. Lisa's physiognomy reflects wood and fire elements, indicating a predominantly yang constitution, characterized by a fast-paced lifestyle and a lack of yin energy. She is experiencing symptoms such as jaw pain, difficulty kissing, tinnitus, and headaches, which are exacerbated by emotional stress related to her personal life, including caring for a handicapped child and navigating a new romantic relationship.
Initially, Lisa sought treatment from her doctor, who recommended Botox injections for her jaw pain. However, after receiving only two injections, she experienced severe headaches without any relief from her jaw symptoms. This prompted her to return to Julie for acupuncture treatment after a long hiatus, having previously been a patient 13 years earlier.
During the intake process, Julie emphasized the importance of understanding Lisa's current life circumstances and emotional state, which informed her treatment approach. The treatment plan involved acupuncture sessions focused on addressing the multifaceted nature of TMJ dysfunction, including both physical and emotional components. Julie recommended one treatment per week, allowing Lisa time to integrate the benefits without feeling rushed.
Over the course of nine sessions, Lisa experienced significant improvement in her symptoms, including the resolution of jaw pain, headaches, and tinnitus. After a two-and-a-half-month break, Lisa returned for a follow-up, reporting that her symptoms had not recurred, except for the ongoing difficulty with kissing, which remained a concern due to her new relationship.
When Lisa was finally approved for additional Botox treatments, she expressed a desire to forgo them, stating that she felt balanced and grounded after her acupuncture sessions. This case study highlights the effectiveness of a holistic approach to TMJ dysfunction, integrating physical treatment with emotional support and patient education, ultimately leading to improved quality of life for the patient.
Techniques and Recommendations
• Motor Points: Explained the significance of targeting specific motor points in the jaw for effective treatment.
• Sound Therapy: Introduced the use of tuning forks to promote relaxation and healing.
• Home Care Recommendations: Suggested exercises, dietary adjustments, and self-care techniques for patients to implement at home.
Challenges and Observations
• Discussed the impact of modern habits, such as phone usage, on jaw tension and overall health.
• Addressed the importance of understanding patients' emotional states and how they manifest physically.
The meeting concluded with a call for participants to engage with the resources shared and consider the holistic approaches discussed for treating TMJ and jaw pain.
