We build and configure a single box solution for home lab pots phones and dial-up internet. We use a Cisco 2811 router and a few easy to find ewic cards.

The following is the complete configuration as dumped with the show run command. There are things here that the router puts here. You only need to copy over the Ethernet, Async (Modem) , Dial peer and the line settings for the modem. Remember that the slot numbers i.e. 0/1/0 for my modem location may be different on your router. This is why I stressed so much how to count the slot locations.
