when these 3 cases happened, you may need to clean your machine;
1) one or two sides of fiber image is very vague;
2) two fibers are mis-aligned;
3) fiber tracking failed;
Use a blade to gently clean the v-groove.
also you could clean the camera with cotton swab and wind cover groove.
