Roadside Chat: Under Miranda, is an Express of Understanding required?

When reading a suspect his Miranda rights, are officers required to obtain an express verbal acknowledgment that the subject understands his rights? If the subject does not respond when asked if he understands his rights, can the officer continue with the interrogation?
