As a long-time pain doctor, I have encountered medial scapular pain that is not related to cervical HCD or stenosis. In these cases, I have observed significant pain relief following dorsal scapular nerve blocks or hydrodissection, particularly when targeting the interfascial plane beneath the levator scapulae muscle or at the nerve's exit point near the posterior tubercle of C5, where it pierces the middle scalene muscle.
Interestingly, I have also noticed similar pain provocation in the medial scapular region during pulsed radiofrequency stimulation at the posterior tubercle region. Some subscribing doctors have proposed that I further explore the dorsal scapular nerve, its role in medial scapular pain, potential entrapment sites, and my treatment strategies.
Therefore, I am committed to studying the dorsal scapular nerve and revealing my treatment strategy. In addition, I will explore the long thoracic nerve, as it exhibits similar characteristics. Stay tuned!
Debate:
How Can the Dorsal Scapular and Long Thoracic Nerves Contribute to Pain Perception or Pain Relief If They Are Purely Motor Nerves?
The dorsal scapular and long thoracic nerves are traditionally classified as purely motor nerves, meaning they lack direct sensory fibers. However, paradoxically, patients with dorsal scapular or long thoracic nerve entrapment or injury often report pain in the medial scapular, shoulder, and anterior pectoral regions. This reality raises an important question:
How can these motor nerves contribute to pain, and why does targeting relieve them?
