We're really pleased to work with Cosine Primary Health Network to continue raising awareness of the great impact of the Comprehensive Primary and Community Care Team roles in General Practice.
The team at Karori Medical is comprised of the Physiotherapist, Clinical Pharmacist and Care Coordinator. Together they see patients as a team to increase the range they can cover in one patient consult.
Meeting unmet needs within General Practice, the CPCT roles have provided wrap-around support for patients and improved long term health outcomes. A great addition to Primary Care.
We hope to see the continued development of the project and thank you to the team at Karori Medical Centre!
