YOU NEED GOOGLE APPS LINK IS ABOVE, JUST DOWNLOAD THEM AND INSTALL THEM VIA TWRP I FORGOT TO MENTION GAPPS SORRY!
1. Download CM12 For Your Device and then put in your download folder on your phone
2. Boot Into Recovery and Backup Your OS and Files and Then Wipe Everything but INTERNAL STORAGE
3. Go To Install and Install the Rom You Downloaded it should be in your download folder
4. boot up and set up the phone and then wallah you are running CM12
