Our Community Inpatient Hospitals care for a range of patients with different needs. We provide support and discharge planning when patients no longer need acute hospital care.
Our rehabilitation units work with you and your family to prepare you for your supported discharge from our community hospital.
Rehabilitation is about making the transition between the dependence of being unwell and in hospital, to being able to manage at home.
There are several options that may be considered for discharge. These will be based on the patient’s current condition and individual needs but may include:
• Home with no formal support or support from family/friends/carers
• Home with short term rehabilitation from the community team
• Home with long term care packages
• Placement to a residential or nursing home
