A 2.5-year-old Chihuahua shows up to the ER after vomiting 18 times, now with bloody vomit (hematemesis), diarrhea, intact female, and a history of struvite crystals. Everyone is thinking the same thing:
GI obstruction?
Pyometra?
Something toxic?
In this video, I walk through the abdominal radiographs step-by-step and explain what I’m looking for, how to identify colon vs small intestine, what pylorus positioning means on different views, and why this case surprisingly does NOT look like a mechanical obstruction.
We also talk about:
When to worry about microhepatica / microhepatia
When bile acids + ultrasound or CT are appropriate
Why supportive care may be the right move
Why recognizing colon first makes abdominal films so much easier
Sometimes the scariest ER vomiting cases… aren’t what you think.

📌 If the dog doesn’t improve in 24 hours → abdominal ultrasound.
📌 No radiographic evidence of pyometra or stones.
📌 Supportive care recommended initially.
