Mid right coronary artery CTO with ambiguous proximal cap, heavy calcification, and bifurcation at the distal cap. Primary retrograde crossing was attempted. Septal collateral crossing was achieved using the surfing technique. The lesion was balloon uncrossable requiring use of the Threader, grenadoplasty, Tornus and laser. Reverse CART was successfully completed using intravascular ultrasound to help with selecting optimal size of the antegrade balloon and verifying proximal true lumen position of the retrograde guidewire. Following wire externalization and stenting an excellent final result was achieved.
