There seems to be an issue going around with PS4s seemingly being bricked out of the box. It's a simple fix and here's how to do it.
If this doesn't work, try the below steps as a possible solution. Keep in mind, this will likely wipe all data from your system.
1. Enter safe mode
2. Rebuild database then let it reboot. (Option 5)
3. Enter safe mode again and update via Internet (option 3).
4. Reboot!
