Sarah and Sam are social workers in an integrated community mental health team. They balance the medical and social aspects of the role.
Although the intersection between health and social care has been complex, they can support the whole journey of someone who might be in hospital, detained under the Mental Health Act, all the way through a Care Act assessment and commissioning a placement for them; this creates a consistency for the users of the service.
