Here is a step-by-step process on how to do this:
Use a PUK Code
• Start by checking the eight-digit code indicated on your card.
• Once you have it, you can enter it correctly when prompted to.
• If this does not work, contact the authority to report this. It should be the one that gave you the card.

At the ID Office
• First, you need to get your second photo ID. You will need it to unblock the card in person. Also, ensure that you are the owner of the card as your presence is required.
• Next, proceed to an ID card office in person. It is also known as the Real-time Automated Personal Identification System or RAPIDS.

Alternatively, go to www.dmdc.osd.mil/rsl Use the site locator to find an office near you.
• While there, go to a Verifying Official. The VO will ask you to provide your fingerprint. You will also need to give them the PIN.
• Next, the VO will verify your fingerprints.
• Upon doing that, the official will then reset your card’s PIN.
