Failed Open Urethroplasty & Penile Skin Flap Surgery

This man had a urethral stricture know as a bulbar urethral stricture. He was treated with numerous dilations, 4 failed laser internal urethrotomy surgeries, and then a failed open urethroplasty using a skin graft from his arm. He then had a penile skin flap surgery. His legs, while in stirrups developed a blood circulation complication called compartment syndrome where large deep cuts were needed to be made in his legs. That surgery failed, and he resumed self-dilation using instruments called filiforms and followers. One day, a filiform became stuck in his urethra and broke off, which would require an emergency room removal. At that point, he was referred to Dr. Joel Gelman, M.D. and underwent a successful re-do re-do urethral reconstruction. At the Center for Reconstructive Urology, custom tables and stirrups are deployed to help prevent blood circulation complications from the legs placed in stirrups, and have never had any complications for compartment syndrome.
