When a patient is diagnosed with scoliosis, they may have been tested or screened several times before they were diagnosed, and many patients don't understand why the scoliosis was found now and not earlier.
Scoliosis screening is important because we know it is progressive. It's in its nature to worsen over time. So the sooner and smaller it is found, the more likely to get a positive outcome, especially when it comes to conservative treatment options.
Scoliosis is most often diagnosed in adolescent cases, and the most common type is idiopathic scoliosis. Idiopathic refers to an unknown cause of scoliosis. Commonly found between 10-18 years of age. Since they are growing, the curve tends to grow with them. How much the curve will progress or how fast is an unknown variable based on how fast the child is growing at this stage.
The most common symptom scoliosis creates in adolescents is postural deviation, such as uneven shoulders, uneven rib arch, and uneven hips, something saying that there is a lack of symmetry in their posture.
It is important to be proactive regarding adolescent idiopathic scoliosis because of how fast it can progress and how bad it can get. In the end stage of scoliosis progression and severity, reaching the point where scoliosis surgery is needed, it is a very invasive treatment. Looking at curves and treating them when they are small can lead to a much better outcome where you can avoid needing such invasive treatment.
Early detection is important because smaller curves and younger patients are much easier to treat as they are much more flexible. Detecting smaller curves before progression and growth, we can use the growth to help correct the spine as we can alter and interrupt the abnormal development and lead to more symmetrical growth.
The most common scoliosis screening is Adam's forward bending. It is when a patient bends forward, and they look at their rib arches. It is sometimes combined with a scoliometer. The issue with common public tests is that since they're all done at the same time, kids don't go through their growth spurts at the same time.The curve needs to be significant to be seen visually without an X-ray. This is why the average size of initial diagnosis is in the 25-35 degree range.
Early detection is definitely beneficial, especially in conservative treatment, as we know proactive treatment when applied early, can make a significant impact on the outcome of the patient. In curves that are less than 25 degrees about a third become severe by the time they finish growing and require surgery. It is important to try to catch the curve less than 25 degrees and try to keep it under 25 degrees. Being proactive as close to your time of diagnosis or if there is a family history of scoliosis is the best advice I can give to any family.
