What is Intralipid Infusion?
Intralipid is an IV-based infusion that contains fat emulsion and is traditionally used as parenteral nutrition for patients in ICUs suffering from severe malnutrition or muscle wasting. It is a 250 ml white-colored solution, photosensitive, and generally administered through a standard IV line in a hospital setting. In fertility treatment, intralipids are believed to work by modulating immune responses, especially acting on uterine natural killer (uNK) cells.
How Intralipid Works?
uNK cells are a special class of immune cells present in the endometrium of the uterus. Intralipids are thought to bind with PPAR receptors on these cells, thereby reducing their cytotoxicity and promoting embryo tolerance. This helps in the early acceptance of the embryo and supports implantation and continued pregnancy. However, this is just one known mechanism and not fully understood. Research is still ongoing.
When Can It Be Considered?
Intralipid infusion should be considered only in carefully selected cases. The patient should have a normal pelvis – meaning no fibroids, endometriosis, or adenomyosis. Autoimmune disorders like rheumatoid arthritis, SLE, APLA, or anticardiolipin antibodies must be ruled out. Most importantly, it should be a euploid embryo, genetically normal, and the pregnancy should be from self egg and self sperm. This is not meant for donor cycles unless there's a very specific reason.
What if Standard Immune Therapies Have Failed?
In some cases, when first-line immunotherapies have already been used – such as steroids, hydroxychloroquine (HCQS), ecosprin, or low molecular weight heparin (LMWH) – and all have failed, intralipid infusion can be considered as an add-on. But remember, it’s “may be considered,” not “must be given.” There is no strong evidence proving intralipids alone improve pregnancy outcomes. Still, in some cases, it has shown statistical benefit.
How Is It Administered?
There’s no universally accepted regime for intralipid use. Some doctors start it 20 days before embryo transfer, similar to IVIG protocols. It may be given weekly or bi-weekly, and typically continued until 10 to 12 weeks of gestation, often ending after a normal NT scan. Some clinicians may stop it after cardiac activity is confirmed. Beyond that, its role remains unclear.
