In this video a patient with cranial nerve III palsy presents for evaluation. Work-up was extensive with a CT scan and MRI/MRA and all was negative. No evidence of an aneurysm was noted. The associated headache was most likely due to the ischemia of the nerve. The underlying and contributing co-morbidity was diabetes mellitus.
