Case / Care Managers provide coordination of care for patients who are managing serious health conditions such as, chronic obstructive pulmonary disease (COPD), diabetes, kidney disease, and congestive heart failure (CHF) to name a few. Case / Care Managers also provide assistance and coordination for many other situations including mental health, long-term care, physical therapy, and other resources that may help patients lower their costs (i.e. prescription drug cost).
This episode explains where one can find Case / Care Managers; your doctor's office, your specialist's office, the hospital and by calling your health insurance company as examples. Case / Care Managers are an advocate for patients and assist to provide coordination between the patient and their Providers to receive the best possible care.
Case / Care Managers and Social Workers also coordinate assistance through the doctor's offices to obtain resources such as oxygen, home safety equipment, wheel chairs, etc. and also help to schedule doctor office visits and follow-up therapy after surgery if needed.
