Documents required :-
Dr. registration certificate with MCI
Owner pen card (organisation pen card)
Amount DD (DRA CE .....
(district name )
Biomedical registration certificate
Barcode implementation
Application on letter head (as need )

All documents submit at CMO office. and take receving recipit.
