Standard 3.3 Survivorship Care Plan | Commission on Cancer Program Standards 2012

Standard 3.3, Survivorship Care Plan, is a phase-in standard for 2015. It requires that all cancer programs provide a summary of treatment and a follow-up plan to all patients completing cancer treatments.

This video series was created to assist you in understanding and interpreting the CoC Standards. The series includes an introduction video, two videos describing the Eligibility Requirements and an additional 26 individual videos describing select standards. Each video begins with the definition of the standard, describes the requirements for meeting the standard and concludes with best practice information.
