What is Stage 3 Ovarian Cancer?
Stage 3 means cancer has spread to the peritoneum or retroperitoneal lymph nodes. The treatment goal is to remove as much disease as possible. This is called “No Visible Disease” or “Optimal Cytoreduction.”

When is Surgery First Better in stage 3 ovarian cancer?
Surgery first is ideal if disease volume is limited and patient health is strong. It requires a skilled surgical team and advanced facilities. This approach can offer better long-term outcomes when conditions are optimal.

When is Chemotherapy Before Surgery Better?
Chemotherapy first is chosen when:
Patient health is weak or there is significant ascites.
Suspected Stage 4 spread.
High disease volume with very high CA 125 levels.
Staging laparoscopy shows widespread disease.
Financial or logistical delays make immediate surgery difficult.

Why This Decision Matters?
Older studies showed similar results for both approaches but had limitations in assessment and surgical standards. Recent studies, like the TRUST Trial, show “surgery first” can provide better disease control in experienced centers.
