Subclinical hypothyroidism is very common nowadays. What is subclinical hypothyroidism? The patient has a lot of features that indicate towards thyroid or a thyroid problem and you may feel that this patient could possibly have a thyroid issue but when you do a thyroid test the values of the thyroid T3, T4 are within the normal range and the TSH in the reports is also in the normal range but that is not an exact way to decide of the patient has a thyroid problem or not. If the thyroid is not functioning properly what we usually asses is T3, T4 and TSH. Thyroid secretes not only T3 and T4. It also secretes other hormones. T0, T1,T2, T3, T4 are the various hormones secreted by the thyroid glands. We normally check only the T3 and T4. TSH or thyroid stimulating hormone is secreted by the pituitary gland. Pituitary gland is the master controller of all the glands in the body. It is located in the head and thyroid is located in the throat. It keeps on secreting thyroid stimulating hormone to stimulate the thyroid if the thyroid is not functioning adequately. So there is always a balance in the body if it its maintained because of the thyroid is not functioning adequately the pituitary goes and reminds it by secreting thyroid stimulating hormone. It stimulated the thyroid and makes it to secrete T3 and T4. There are two types of thyroid issues, one is a primary thyroid problem, that is the problem is with the thyroid glad and secondary thyroid problem means due to some other factor that is controlling the thyroid and because of which the thyroid is not able to secrete T3 and T4. In primary hypothyroidism the T3 and T4 is less because the thyroid is not producing enough T3 and T4. In such cases the pituitary keeps remaining the thyroid by secreting more thyroid stimulating hormone to release more T 3 and T4. What is the normal range for TSH? 0.3 to 5.5 in the earlier reports it used to be mentioned as the normal range, nowadays it is found that 0.3 to 4.2 is considered as the normal range, I would say healthy thyroid functioning is 2 to 4 would be not too alarming. When you find the value is going more than 4.2 the you need to be something concerned about. That means there is an excessive TSH being secreted and the person has associated thyroid symptoms then that means there is an excessive of TSH being secreted and if thee patient has associated thyroid problem symptoms then probably the thyroid is not functioning, it is subclinical means it is having all the symptoms, clinically we cant call it as a thyroid issue, but it is subclinical indicating a thyroid problem. There usually artificial T3, T4 may not be started. Same way TSH can be less than 0.3 it usually happens in secondary hypothyroidism where pituitary is not producing enough SH. So when the pituitary is not producing enough TSH over a period of time then thyroid also fails to produce enough T3 and T4. So that is secondary hypothyroidism and in such patients the value is within the normal range and that is subclinical hypothyroidism because all the systems are there of thyroid but when you clinically you have to call it thyroid, it is out of the normal range, maybe in the upper limit or the lower limit, and yet you have thyroid features, that is when you call it as subclinical hypothyroidism. Some ask my value is within normal and why do I need to start medicines? That is in allopathy where we don’t starts any medicine because artificial T3 and T4 is started only when the values are very ow and the thyroid is not functioning well. Then it is artificial T3 and T4. So in homeopathy itself starts releasing thyroid hormones and the pituitary functions better. I had a patient whose thyroid values or TSH was around 0.1 but T3, T4 was normal so they didn’t want to start any medicines in allopathy. Pituitary function the prolactin was going higher. So in that case it indicates that the pituitary is not function; so there was certain medicines made out of pituitary gland extracts. So pituitarinim is the name of the medicine and the patients pituitary function started improving, prolactin started becoming normal and TSH became normal. It is possible in homeopathic medicines. So there is no reason for anyone to feel I have subclinical hypothyroidism and we can improve the function.
