This review article, published in the Indian Journal of Ophthalmology, explores the history and evolution of the use of botulinum toxin in ophthalmology over the past five decades. The article highlights how botulinum toxin, a neurotoxin that blocks muscle signaling, has evolved from a therapy for strabismus to become the gold standard for treating a wide range of ophthalmic conditions, including blepharospasm, facial hemispasm, facial paralysis, spastic entropion, strabismus, endocrine orbitopathy, convergence spasm, and facial trauma. The article also examines biosimilars of botulinum toxin type A, their applications, and potential side effects. Finally, the article presents future perspectives on the development of non-injectable administration routes and long-acting forms of botulinum toxin.
