The basic idea is to stack physical defense, maximize soul income per boss, and then buff a pair of Sellswords Twinblades or Warden's Twinblades, with either Carthus Rouge or your buff spell of choice. I also frequently used the Red Hilted Halberd against a wide variety of enemies, especially NPC invaders. And of course, relying heavily on Tears of Denial and Hidden Body.
EDIT: NG+ actually goes to +7 in this game. Wish they'd stop changing it! Information should still be good.
