Ohio waiver services serves individuals with limited incomes and resources and is a very cost effective program in comparison to nursing home care. The services include:

Adult Day Health
Emergency Response Systems
Personal Support Service, which includes assistance with bathing, light housekeeping, laundry, shopping, meal preparation and respite
Home Delivered Meals
Skilled Nursing Service
Out of Home Respite Care
Alternative Living Services, which provides meals, personal care and supervision in an approved personal care home.
Services are provided through a network of providers.
New Providers
Providers of services to people with developmental disabilities may be self-employed individuals (independent providers) or agencies.
Because the health and safety of people accessing services through the Ohio Department of Developmental Disabilities (DODD) is important to us, DODD certifies both agency and independent providers of services.
An individual or agency is prohibited from providing any service until certification is obtained from DODD
Important Things to Know

• A birth certificate is a required document and will need to be submitted as part of your application.
• It is recommended that applicants first gather all documents (except the BCII) prior to completing the application. It is also recommended that you upload your documentation when the application system prompts you.
• The BCII background check must be received directly from the Ohio Attorney General’s office.
• Please be aware that during the review process, you may be asked to provide additional documentation.
• Once your fee has been paid and your BCI background check has been received, your application will be submitted for review. DODD will review you application within 30 days of this submission date.
• Following the review of your application and documentation by DODD, your information will be submitted to Medicaid for approval and issuing of a Medicaid number.
• For new agency applicants: Medicaid will likely require and conduct a site visit of your agency prior to the approval of a Medicaid number. A change affecting applicants to be independent providers of transportation services:

• If you are applying for a transportation service, you will need to provide an official driver’s abstract prepared by the Ohio BMV.
The BMV reports on its website that the bureau will no longer provide an unofficial driving record (abstract) as a printable document. The BMV currently only allows a screen view of a person’s official driving record, but it contains no unique, identifying information. As a result, this unofficial driving record no longer meets the requirement of rule, which requires applicants of transportation services to submit a driving record (abstract) prepared by the BMV to the Department of Developmental Disabilities.
Please keep in mind that rule requires a driving record (abstract) be prepared by the BMV no earlier than 14 calendar days prior to the date of the application.
