Causes of lean condition trouble codes P0171, P0174 - GM 5 3L

The vehicle being worked on is a 2004 Chevy Suburban with a 5.3L engine. It had multiple trouble codes in memory P0171 and P0174 (fuel system too lean bank 1 and 2) a P0101 (MAF circuit out of range) and a P0300 (engine misfire detected). All of these were related to one single problem.

Symptoms
rough idle, especially cold
check engine light (P0171, P0174, P0101, P0300)
poor gas mileage
hesitation on acceleration
runs much better with engine warm

Fix
replace intake manifold gasket
