👶 តើទារកបែបនេះមានលក្ខណៈដូចម្តេច?
TRAP Sequence បង្កើតទារកមួយ (ហៅថា acardiac twin) មានលក្ខណៈខុសប្រក្រតីយ៉ាងខ្លាំង៖
1. /មិនមានបេះដូង (acardiac)
→ មិនអាចបូមឈាមឲ្យខ្លួនឯងបានទេ
→ ឈាមបានមកពីភ្លោះម្ដងទៀតតាមរបៀបបញ្ច្រាស (reversed flow) តាមសរសៃ Umbilical artery ។
2. /ក្បាល និងខួរក្បាលមិនអភិវឌ្ឍ ឬមិនច្បាស់ ៖
→ ជារឿយៗ គ្មានក្បាល គ្មានសួត គ្មានសរសៃប្រសាទកណ្តាល
→ មានតែផ្នែកខាងក្រោមដូចជា ជើង, ត្រគាក, បំពង់នោម… ដែលអភិវឌ្ឍជាគ្រោះថ្នាក់ ។
3. /មើលទៅដូច “mass” ឬ “tumor” នៅក្នុងសំប៉ែត៖
→ មានទ្រង់ទ្រាយក្លែងក្លាយ (amorphous structure), មិនដូចទារកធម្មតា។
4. /គេបែងចែកជា ៤ ប្រភេទ acardiac twin៖

៙Acardius acephalus ខ្វះក្បាល, មានតែជើង–ត្រគាក
៙Acardius anceps ក្បាលមិនច្បាស់, សាច់ដុំបែកបាក់
៙Acardius amorphous មិនមានទម្រង់ទៀងទាត់, ដូច mass
៙Acardius myelacephalus មានផ្នែកខាងក្រោយជាចំណែកspinal cord ។

Ultrasound (អេកូសាស្ត្រ) បង្ហាញថា៖
• ភ្លោះមួយឃើញដូចទារកធម្មតា
• ភ្លោះមួយទៀតមិនឃើញបេះដូង, ឬក្បាល បែបក្លែងក្លាយ
• Doppler ultrasound បង្ហាញការហូរឈាមបញ្ច្រាស
🩺 តើព្យាបាលដូចម្តេច?
• តាមដានជិតស្និទ្ធ
• បើ acardiac twin ធំ (ខូចខ្លាំង) → គេអាចប្រើរបៀប radiofrequency ablation, laser coagulation ដើម្បីបិទសរសៃឈាមទៅវា
• គោលបំណងគឺជួយសង្គ្រោះ pump twin (ទារកសុខ)

Twin Reversed Arterial Perfusion (TRAP sequence) is a rare condition that occurs in twins (usually seen in monozygotic, monochorionic twins). In this condition, one baby is severely damaged and receives blood from the other twin in the opposite direction — without its own heart to pump blood.
👶 What does this baby look like?
TRAP Sequence creates a baby (called an acardiac twin) with very different characteristics:
1. /No heart (acardiac)
→ Unable to pump blood for itself
→ Blood is returned from the other twin in a reversed flow through the umbilical artery.
2. /Head and brain are underdeveloped or not well developed:
→ Often no head, no lungs, no central nervous system
→ Only the lower limbs, such as the legs, pelvis, and urethra, develop abnormally.
3. /Looks like a “mass” or “tumor” in the abdomen:
→ Has an amorphous structure, unlike a normal baby.
4. /They are divided into 4 types of acardiac twins:
Type Description
Acardius acephalus lacks a head, only legs and hips
Acardius anceps has an unclear head, broken muscles
Acardius amorphous has no regular shape, like a mass
Acardius myelacephalus has a back part that is part of the spinal cord.
⚠️ Problems that occur with the other twin (pump twin):
• The healthy twin (pump twin) must pump blood from itself to the defective twin
• This creates a heavy load on the heart → heart failure
• Can lead to death (1 in 2 cases) if left untreated.
🧪 What is the diagnosis?
Ultrasound (echo) shows:
• One twin looks like a normal baby
• The other twin does not see the heart, or the head is false
• Doppler ultrasound shows reverse bleeding
🩺 How is it treated?
• Close monitoring
• If the acardiac twin is large (severely damaged) → Radiofrequency ablation, laser coagulation can be used to close the blood vessels to it
• The goal is to save the pump twin (healthy baby)
