3- IF broken again, the consequence is initiated

Don't make agreements you can't/won't keep. Get good at finding solutions.

Figure out what you need to trust again. What actions, words and feelings do they need to express for you to feel like you could trust fully - without bringing up the transgression up as ammunition.
