Septic shock has the highest case fatality rate of all conditions affecting children in PNG, the mortality rate exceeds 50-70% in all hospitals. Septic shock is often associated with infection in other sites (pneumonia, empyema, malaria, UTI, gastrointestinal, meningitis), or with preceding virus infections. Early recognition and treatment greatly improve outcomes, but that management is multi-faceted. We will discuss the causes of septic shock, how these causes are changing with antimicrobial resistance, and we will discuss the emergency treatment and ongoing care of such critically ill children, and what to do when sepsis is poorly responsive to initial therapy.
