Heart failure in children can be caused by congenital or acquired conditions, and there is a characteristic presentation and a differential diagnosis of each cause. By understanding the clinical patterns of heart failure presentation at different ages, we can make diagnoses based on history, examination and simple tests (chest x-ray, ECG). In many children with heart failure there is the underlying cause and a precipitant that lead to decompensated heart failure, and we need to look for, recognise and treat the precipitants if the heart failure is to improve. The precipitant could be an infection, arrhythmia, anaemia, non-adherence with anti-failure medications. The diagnostic process must uncover the precipitating factors as well as the underlying cause of the heart problem and treating these has to be part of the management plan.
