Steroid-Responsive Myorhythmia in the Pectoralis Muscles in a Patient With Anti-Tr Antibody

A 74-year-old man presented with involuntary and painful contractions in his right pectoralis muscles for about 6 months. The symptoms persisted predominantly at rest, disappeared during sleep, and could be relieved by active muscle forcing. The involuntary contractions could be observed in both the pectoralis major and minor on B-mode ultrasonography. Electromyography of the pectoralis major showed 1.5-Hz rhythmic discharges of motor units, each lasting approximately 250 milliseconds, which was consistent with the characteristic of myorhythmia. Anti-Tr antibodies were identified in his serum by immunoblot. The involuntary movements of the pectoralis muscles were apparently alleviated after steroid treatment. Myorhythmia might be a rare clinical manifestation of anti-Tr antibodies, which warrants further investigation in the future.
