This montage occurs as Jennifer sits by Jack's bedside in what she believes to be one of his last days. If you want to skip the Jennifer and Frankie stuff, go to 1:00.
This is after he's returned from a seven month absence during which he was presumed dead. His car had plummeted off a bridge into freezing waters, but he'd decided after he swam to the surface and was rescued by a fishing trawler that instead of returning to his family, he'd allow them to believe he'd perished in the accident. He'd wanted to spare his family the pain of watching him die slowly from his terminal illness which was then just beginning its final stages. However, now he's back, and it really does appear to be the very end.
He'd returned right at the end because while at a hospital, he'd come across his long-lost presumed dead brother Steve! He'd brought Steve back to Salem just in time to see Jennifer marry Frankie. Yeah, it took all of seven months for her to remarry... At least "there's hope a great man's memory may outlive his life half a year!"
To be fair, Jack had pushed absurdly hard for Frankie to romance Jennifer after he died, and everyone else in Salem had been reinforcing their pairing with the exception of Abigail. Jack had even actively pushed them together before he'd supposedly "passed away." Still, Jack had been unprepared to walk in on their wedding.
Jack had gotten over his shock quickly, though. After all, he was still dying, and this had been his idea. He'd told them he was delighted his plan had worked so well, and that he only regretted spoiling the wedding by showing up. He'd even apologized to Frankie! He'd then tried to make himself scarce again since he'd fulfilled his obligation of delivering Steve to Salem. However, they'd insisted he stay this time, and he'd gone home.
At this point, Jennifer wants to care for Jack as he endures the worsening pain. It doesn't take long for her to see how bad it's gotten. After one particularly harrowing bout of Jack suffering Ivan Ilyich levels of agony, we get this montage.
We all know Jack survives this. You may wonder how. Well, while Jack is suffering horribly and Dr. Carver is providing little more than sympathy, Frankie is searching the internet! He finds a clinical trial of an experimental drug that just happens to be run by someone he knows, and he gets Jack in at the very last minute. The medicine cures him... and this excruciating storyline finally comes to an end!
