When scoliosis is diagnosed, what defines the diagnosis is the area of the spine where the scoliosis is found. This is done by identifying the 3 main sections of the spine:
Cervical-neck
Thoracic-midback
Lumbar-lower back
Scoliosis is when the spine shifts out of its normal alignment, an abnormal sideways curve of 10 degrees or more with some form of twist of rotation. Lumbar scoliosis is when scoliosis affects the lumbar portion of the spine. It is only a certain section of the lumbar spine, this section is identified by the apex of the scoliosis, which is the most horizontal section of the curvature. For lumbar scoliosis, the apex is located at L2, L3, or L4.
The most common identifiers of lumbar scoliosis are seeing one side of the waist look flat and the other curved, as well as a tilting of the torso over the hips.
Lumbar scoliosis is the most painful, from the low back, to the leg, and sciatic pain that can spread down to the patient's legs. This is because, in this section of the spine, scoliosis affects the muscles, nerves, and tissues around that area. Especially sciatica is more common in adult lumbar scoliosis patients.
Treating lumbar scoliosis with the traditional approach involves the "watch and wait" approach, bracing, or scoliosis surgery. As scoliosis progresses for adults it is more likely to cause pain.
For patients who choose surgery, there is a tradeoff and the results of spinal fusion in adults are not great. Any non-invasive, more natural approach should be the way to go.
Dr. Tony Nalda recommends adult patients with lumbar scoliosis first to assess where they are at with their scoliosis, measuring the size and the apex of progression. Second, develop a completely customized plan for their scoliosis. And lastly, coordinate those treatments properly, make sure the care is cohesive and not fragmented.
