Leptospirosis is a zoonosis, and an example of ‘One Health’. It is not a diagnosis we make often, however the pathogen is found in all countries in Oceania. Recent large outbreaks in Fiji affecting children and adults, ongoing infections in the north of Australia, and evidence of infections in humans and animals in at least 13 Pacific countries indicates it is more common than we realise. We should think of leptospirosis as a cause of fever in children under certain conditions: floods, exposure to contaminated water and rodents, a child with fever, jaundice, conjunctival haemorrhages, thrombocytopenia, unexplained sepsis. The clinical features of leptospirosis overlap with typhoid and dengue, among other infections. Fortunately, the infection responds well to antibiotics if treatment is started early. We discuss the clinical features, diagnosis, treatment, and supportive care of children with leptospirosis.
