This workplace incident recreation illustrates how a worker was struck and killed by a pipe while venting a CO2 tanker at a natural gas wellsite.

This video recreates the steps leading up to the fatal incident showing what caused a ventilation pipe to blow off of a tank known as a queen unit. The video also shows steps the employer took to ensure this risk was mitigated in the future.
