For any new viewers, I wanted to introduce myself! I'm Prerak - a third year internal medicine resident at Stanford! As mentioned above, this video provides a detailed overview of H. pylori, which is a significant infection within the stomach. This is something that affects a lot of people and there are more and more antibiotics that the bacterium is resistant to. This is why knowing how to treat this is so important.

H. pylori is a motile, gram negative organism; has urease (breaks down urea into ammonia and carbon dioxide), which is why you can use urease breath test for diagnosis (albeit, gold standard is stool antigen; (95% sensitive, 97% specific)). Additionally, one of the most common sign of H. pylori infection: Duodenal ulcers. Additionally, chronic H. pylori infection with gastric malignancy (gastric cancer) + mucous associated lymphoid tissues

04:11-05:15 - Increasing Resistance Rates for H. Pylori.
Rising rates of resistance to key antibiotics used to treat H. pylori, including clarithromycin and levofloxacin. This has led to reduced effectiveness of commonly used treatment regimens that contain these antibiotics.
05:18-07:01 - How to Treat H,. Pylori?
Initially there was this regimen: Original regimen (2017): Clarithromycin, PPI, Amoxicillin OR Metronidazole (Bismuth quadruple therapy was also considered first line if c/f prior macrolide exposure)
However, now there is a Bismuth quadruple therapy regimen.

Vonoprazan - U.S. FDA-approved recently to treat erosive esophagitis, but not gastroesophageal reflux in the absence of esophagitis. Also approved for H. pylori.
