What does #valuebasedhealthcare look like clinically at the point of care⁉

Now I have presented on value based care in kidney disease numerous times in the past. I have literally done a 20 minute lecture presentation going over the new payment models that Medicare came up with.

But as a clinician, administrator, technologist or even patient, what should you expect.
Here is my brief checklist, let me know what you think.

✔Screening for kidney disease
✔Early referral to a #nephrologist
✔Slow down progression of kidney disease

✔Diet and lifestyle modification
✔Blood pressure control (ACE inhibitors or ARBs)
✔Blood sugar control (SGLT2 inhibitors)

✔Decrease hospitalizations
✔Improve medication reconciliation
✔Patient education

✔Decrease "crashing" into dialysis
✔Increase home #dialysis rates
✔Early referrals to kidney transplant centers
