There are some nerve related bladder problems which can pose a risk or a threat to the kidneys. We call these conditions as NE, high risk neurogenic lower urinary tract dysfunction or in lay parlance, high risk neurogenic bladder. Some examples of these conditions are spinal cord injury, spina bifida, anorectal malformation and some other associated conditions. Unfortunately, in our country, spina bifida and other birth-related spine abnormalities remain commoner than in the Western population for various reasons. We don't have folate supplementation. We don't have universal screening of these pregnancies for neurological conditions, and hence, there's a higher prevalence of spina bifida in the Indian population. If these children are not evaluated and treated in time, then they can damage their kidneys, and this is a double disaster. A child born with a spine abnormality which might impact their lower limbs might put them into a wheelchair, now has a preventable problem which should never have happened in the first place where the kidneys are affected. However, if these children are evaluated and treated in time, it should always be possible to prevent kidney damage in such children. Some children with spina bifida can damage their kidneys very rapidly, even in a matter of months or a year or two, and hence it's important to explain to such families that the bladder needs to be assessed and managed at the earliest. Typically, this should happen within days of the spinal cord surgery to take care of the underlying problem. And in many such children, treatment would actually be lifelong. But as long as they manage correctly, it should be possible to rehabilitate them. And they can be good functioning members of our society with long term health of the kidneys and the urinary tract. Such management can ensure that they don't leak urine, they don't leak feces, that they have good personal hygiene. and that it should be possible for them to attend school and interact with their peer group, all of which becomes a challenge if they have problems related to urine or bowel control. With good management, most such children can have a normal lifespan and a good quality of life. "
